- Review and organize all medications, evaluating possible conflicts and side effects, and develop a medication list to be shared with all providers.
- Review and organize medical records to limit risk of medical errors.
- Prepare you for a medical appointment or test.
- Assist with Advance Directives and health care proxy paperwork.
- Relay health care information to caregivers and loved ones.
- Help you to become your own health care advocate.
- Accompany you to appointments and therapy sessions as needed (this service is subject to temporary suspension relative to Covid-related risks and restrictions).

What I Don't Do
- Diagnose or treat illnesses.
- Prescribe medication.
- Provide medical advice. That advice should come only from your physicians. I will, however, help you to understand and implement that advice.

- Provide transportation to/from medical appointments.
I will inform you, guide you, and support you in your health care decisions, but I will not make choices for you.
